Printing Instructions
  1. Load the pre-printed hospital consent form into your printer tray
  2. Open the downloaded PDF and press Ctrl+P (or Cmd+P)
  3. Set scale to 100% (do not use "Fit to page")
  4. In Chrome: More settings → Background graphics: OFF
  5. Print on the pre-printed paper
The PDF contains only text — no background image is embedded, so it is safe to print even without changing print settings.
